Ten pages, two formulation bases and five ready formulas for one of the shortest ingredient lists in personal care. This is ORIZI Group’s 2027 nipple balm proposal walked page by page — the shelf economics, the two bases, the exclusion list, the claim wording Malaysia allows, and the three formats they run in.

Disclosure: This article is published by ORIZI Group and presents our own OEM development proposal. Every item shown is a formulation concept for private-label development, not a finished registered product. Register figures were read from the NPRA cosmetic notification database (QUEST3+) in October 2026. Claim wording is finalised against the destination market’s regulatory route before artwork goes to print.
Quick answer: what is in the 2027 nipple balm proposal?
The proposal covers one product category — nipple care for breastfeeding mothers — on two formulation bases: a pharmacopoeial-grade lanolin base and a vegan candelilla base built from shea, cocoa and mango butters in olive oil, with no lanolin and no beeswax. Both are anhydrous, so neither carries a preservative system. Five ready formulas run across three formats: tube (7 g / 15 ml / 40 ml), 60 g jar and 15 g twist-up stick. In Malaysia the category is a notified cosmetic (NOT) through NPRA, and the claim wording on pack is what decides whether it stays one.
Key takeaways
- The shelf pays well for a very simple product. A 40 ml tube sells at RM54–82 in Malaysian pharmacy retail, for a single-ingredient or six-ingredient anhydrous balm.
- The category is filling up fast. 16 of the 27 nipple products on the NPRA register arrived in the last 22 months; it took eleven years to reach the first eleven.
- Two bases cover two different shoppers from one development run — lanolin for the traditional, hospital-bag buyer; vegan candelilla for the clean-label, Halal and lanolin-avoiding shelf.
- The exclusion list is the product. No water, no preservative, no fragrance, no essential oils, no colourant, no petrolatum — which is what makes “no need to wash off before feeding” defensible.
- One word moves you to another regulator. “Helps prevent cracked nipple” is a cosmetic claim; “for use on cracked or sore nipple” is not.

Page 02 — Why this product, why now

Mothers do not shop around for this one. They buy the tube they trust, they buy it in the hospital bag, and they buy it again for the next baby. That repeat behaviour is why a product with six ingredients or fewer holds a pharmacy price most body-care formats never reach.
| Number | What it is | Why it matters |
|---|---|---|
| RM54–82 | Shelf price for a 40 ml tube in Malaysia (Lansinoh, Medela, observed retail, October 2026) | Gross margin headroom on a single-ingredient, water-free balm |
| 16 of 27 | Nipple products notified in Malaysia in the last 22 months, out of 27 on the register | Demand is proven and entrants are still arriving |
| 6 or fewer | Ingredients on the label | No emulsion, no preservative system, short stability path |
| 1,000–10,000 | Monthly searches for “nipple cream” in Malaysia (Google Keyword Planner, October 2026) | The shelf word is cream, even for a balm — name the product accordingly |
Almost all of the new names on the register are small private-label brands with a factory behind them. That is the job this proposal does.
Page 03 — Your brand, our balm

The product is unbranded. You bring the pack, the name and the positioning; we bring a balm that is thick enough to stay where it is put, soft enough to apply every day, formulated so nothing needs to be wiped off before the next feed, and short enough that a first-time mother can read the whole label in five seconds and trust it.
Page 04 — Two bases, two shelves

This is the decision that shapes everything else on the pack. The two bases are not better and worse; they sell to two different shoppers standing in front of the same category.
| Base L — Lanolin | Base V — Vegan & Halal | |
|---|---|---|
| Architecture | Single-ingredient anhydrous balm | Shea, cocoa and mango butters in olive oil, set with candelilla wax |
| Grade control | USP Modified / EP grade, pesticide and free-alcohol controlled, evidenced lot by lot | Plant-derived throughout — no lanolin, no beeswax at all |
| Skin feel | Thick, protective, familiar | Lighter, nicer for daily use |
| Sells on | Trust and tradition — the hospital-bag and midwife favourite | Clean label and Halal — vegan, allergy and clean-beauty shelves in one |
| Ready formulas | Lanolin Nipple Cream · Spreadable Lanolin Nipple Cream | Vegan Nipple Balm (Candelilla) · Vegan Nipple Butter (Jar) · Nipple Balm Stick |
Launching both is cheaper than it sounds: same artwork family, same pack, same development run — only the base changes. The traditionalist takes the lanolin tube, the clean-label mother takes the vegan one, and both are your customers.
One point worth being precise about: a large share of balms sold as lanolin-free still contain beeswax, which is animal-derived. The vegan claim on the front of pack quietly fails, and the Halal file gains an origin question. Candelilla wax (INCI: Euphorbia Cerifera Cerifera Wax) removes both problems. It is harder than beeswax, so it is not a 1:1 substitution — expect roughly 60–70% of the beeswax quantity, confirmed on the bench.
Page 05 — What is inside

| Ingredient | Role |
|---|---|
| Olive oil (Olea Europaea Fruit Oil) | Base liquid oil |
| Shea butter (Butyrospermum Parkii) | Body and skin feel |
| Cocoa butter (Theobroma Cacao Seed Butter) | Firmness |
| Mango butter (Mangifera Indica Seed Butter) | Non-greasy finish |
| Candelilla wax (Euphorbia Cerifera) | Plant wax structure — sets the format |
| Sunflower oil (Helianthus Annuus Seed Oil) | Softens the draw |
| Calendula (Calendula Officinalis Flower Extract) | Signature botanical |
| Vitamin E (Tocopherol) | Antioxidant — protects the oils in place of a preservative |
Three exclusions carry the positioning. No water, so there is nothing for microbes to grow in and no preservative on your label. No beeswax — the quiet ingredient that breaks a vegan claim on competitor packs. No fragrance, because nothing is added for the shelf that a baby would taste at the next feed.
Page 06 — The line that sells it

This is the sentence the category is bought on, and it is only as good as what you leave out. The exclusion list behind it is absolute: no fragrance, no essential oils, no preservative, no menthol, no colourant, no petrolatum. Every ingredient is selected on the assumption that residue may be ingested by the infant — which is the standard this product is designed to, not a marketing posture.
Page 07 — One word decides who regulates you

Nipple care in Malaysia is a notified cosmetic. Notification (NOT) is a self-notification through NPRA’s QUEST3+ system, held in your company’s name, with a safety assessment and product information file behind it. The same fork decides the route for scar gels, acne patches and barrier creams — we mapped it in how Malaysia classifies cosmetics, drugs and medical devices.
| Print this | Never print this |
|---|---|
| Helps prevent cracked nipple | For use on cracked or sore nipple |
| Moisturising | Heals · repairs · treats |
| Protects and conditions the nipple area | Forms a layer to promote healing |
| Fragrance-free · preservative-free | Antibacterial · anti-inflammatory |
The right-hand column turns a cosmetic into a medical device — a different authority, a different file and a different year. In practice that is the single most expensive mistake in this category, and it is made in copywriting, not in the lab. We mark up artwork line by line, in writing, before anything goes to print; we prepare the safety assessment, stability and microbiological file behind the notification; and the notification is filed in your company’s name, with the NOT number on your pack.
Page 08 — Three formats, one development run

| Format | Size | Role on shelf | Ready formula |
|---|---|---|---|
| Tube | 7 g · 15 ml · 40 ml | Hospital bag at 7 g; 40 ml is the repeat buy | Lanolin · Spreadable lanolin · Vegan candelilla |
| Jar | 60 g | Butter format for daily nursing care | Vegan Nipple Butter |
| Stick | 15 g twist-up | Travel and hygiene — applied without fingers | Nipple Balm Stick |
Format is set by wax load, not by a new formula. Soft or firm, the same base runs in a tube, a jar or a stick with no reformulation — and all three use standard tooling that is ready to run, so this is a volume exercise rather than a sampling one. We do not offer sachets in this category.
Page 09 — A certified cosmetics factory, not a blending room

For something a baby will taste, the certificate wall is the specification.
| Certification | What it covers |
|---|---|
| ISO 22716 Cosmetic GMP | Manufacturing, documentation and traceability at Dr Orizi |
| NPRA GMP | Licensed manufacturing oversight |
| JAKIM Halal | OEM plus 109 certified products — the default on our line, not a certificate chased for one customer |
| US FDA MoCRA | Facility registration for US-bound export |
| SEDEX / SMETA | Social compliance audit for retail and export buyers |
Five plants in one group cover cosmetics, nutraceuticals and medical devices, so a brand that starts with a nipple balm can extend into the rest of a mother-and-baby range without changing supplier.
Page 10 — Start here

Send us the base you want, the pack size and the market. What comes back is a sample off that base, an artwork mark-up checked line by line against the claims guideline, and a notification plan — from a factory that already makes this every week.
What can be customised, and what cannot
Pack size, pack format, the signature botanical, the butter ratio and the whole of the artwork and positioning are yours to set. Three things are bounded by the product rather than by preference: the base cannot carry water without acquiring a preservative system; the wax load cannot drop below what the chosen format needs to hold its shape in Malaysian ambient temperature; and no claim goes on the pack that the file behind it does not support.
How the claims are substantiated
Evidence runs in tiers. Stability and compatibility testing on the finished pack comes first, then microbiological testing on the finished lot — carried out even though the base is anhydrous. Safety assessment is prepared against the ingredient dossier for the product information file. Prevention and moisturising claims are supported at that level; anything reading as treatment is not substantiated here and is not offered, because it belongs to a different regulatory route.
Which base to start with
If the brand sells through pharmacy and hospital channels, or competes directly against the global benchmark, start with lanolin — the shopper is looking for the format they were told about. If the brand is clean-beauty, vegan, Halal-led or e-commerce first, start with the vegan base, and say what the product is rather than what it avoids. If the range is going to carry both, run them together: the artwork family and the pack are shared, so the second SKU costs far less than the first.
The ORIZI perspective
The failure pattern in this category is never the formula — it is the artwork. A brand owner writes “soothes cracked and sore nipples” because that is what the customer actually feels, the artwork goes to print, and the product is now describing a medical device with a cosmetic notification behind it. The fix costs a print run and a relaunch window. That is why the claim mark-up happens before the sample is signed off here, not after.
The second pattern is the beeswax one. A range launches as lanolin-free, a reviewer reads the INCI, and the vegan claim on the front of pack is publicly wrong. The ingredient was chosen for structure, not for positioning, and nobody checked the two against each other.
Risks, limitations and claim notes
- Every product in this proposal is a development concept, not a stocked finished good or a registered product.
- Register counts were read from NPRA QUEST3+ in October 2026 and change as notifications are added or lapse.
- Shelf prices are observed Malaysian retail at the time of writing and are not a recommended resale price.
- Claim wording differs by market. The table above reflects Malaysia’s cosmetic claims guideline; export markets are reviewed separately before artwork is finalised.
- Lanolin is an animal-derived ingredient. Halal status is handled by source documentation; brands wanting to avoid the question entirely should use the vegan base.
Frequently asked questions
Do I need NPRA notification before selling a nipple cream in Malaysia?
Yes. Nipple care is a cosmetic, and every cosmetic sold in Malaysia needs a notification (NOT) number before it goes on sale. It is a self-notification through QUEST3+, held in your company’s name, with a safety assessment and product information file behind it.
Can I say my nipple cream heals cracked nipples?
No. “Helps prevent cracked nipple” is acceptable cosmetic wording; healing, treating or “for use on cracked or sore nipple” is not, and moves the product to the Medical Device Authority’s route under Act 737.
Does a nipple cream need a preservative?
Not in this architecture. Both bases are anhydrous, so there is no water phase to support microbial growth. Tocopherol protects the oils, and finished-lot microbiological testing is still carried out.
Is the vegan base really beeswax-free?
Yes. Structure comes from candelilla wax, a plant wax. There is no beeswax and no lanolin in Base V, which is what allows vegan and Halal positioning to hold together on the same pack.
What is the minimum order quantity?
Ready formulas start from 500 units per SKU in standard packaging. Custom pack tooling or a reformulated base requires a larger run; we confirm the actual figure once the pack is chosen.
Should I call it a nipple cream or a nipple balm?
Commercially, cream. Malaysian search demand sits on “nipple cream” at 1,000–10,000 a month while “nipple balm” is under 100, even though the product is technically a balm. Use the shelf word in the product name and the technical word in the description.
How long does it take to launch?
From a confirmed base and pack, allow roughly three to four months: sample and sign-off, stability and microbiological testing, safety assessment and file preparation, notification, then artwork and production. Custom bases or imported tooling extend that.
